Drug Price Reversal between Original and Generic Drugs and the Price Reduction Mechanism

Abstract
A drug price reversal occurs when the prices of generic drugs are higher than those of original drugs. This study analyzes the current status of drug price reversal and the drug price reduction mechanism that has the greatest impact on it. We used the Health Insurance Review and Assessment Service's database. The top 5 drug categories with the highest number of drug price reversals were established, and the drug price reduction mechanism applied was identified. We determined the drug price reduction mechanism that showed the highest drug price reduction rate in price cut of branded drugs. There were 275 (29.4%) codes with price reversal in 934. The average number of generics in the active ingredient group where price reversal occurred was 32.7, which was significantly greater than the 11.6 in the non-occurring group. In the drug price reversal group, the average number of price reductions for the original drug was 8.9, more frequent than the 5.2 times in the non-price reversal group (p<0.001). At the time of price reversal, the price reduction mechanism was mainly by actual transaction price survey (antihypertensives: 76.3%), and the most mechanism applied when the price reduction rate was highest was generic listing (antihypertensives: 39.5%). The price reduction mechanism by the actual price survey had the greatest impact on the drug price reversal, and the drug price reduction rate was influenced by the drug price cut following generic drug listing. This study determined the post-management mechanism that causes the price reversal.
